St.Catherines welcomes new patients, we have a straight forward registration process during which you will be offered a general health check-up.Please check that you live within the practice area, referring to the map on the welcome page.

Either telephone the surgery to make a new registration appointment or call in and speak to a receptionist. You will be required to complete a registration form and ethnic monitoring from. All registration applications will need to be accompanies by 2 forms of identification (1 needs to have have a photopgraph i.e passport, driving licence and 1 needs to have confirmation of residency). This will enable us to check your entitlemnt to NHS services.

Our welcome pack includes a copy of the practice leaflet and practice charter. During the registration process you will be asked to complete a registration form for each family member including children.

Following registration your previous records will be requested, there is normally a delay of 4 to 6 weeks before thay arrive, except if urgent.

The accuracy of information you provide us with during registration is therefore very important.
