New Patient Registration

register_now_pink_signSt.Catherines welcomes new patients, we have a straight forward registration process during which you will be offered a general health check-up. Please check that you live within the practice area, referring to the practice map.

Either telephone the surgery to make a new registration appointment or call in and speak to a receptionist. You will be required to complete a registration form and ethnic monitoring from.

All registration applications will need to be accompanied by 2 forms of identification (1 needs to have have a photopgraph i.e passport, driving licence and 1 needs to have confirmation of residency). This will enable us to check your entitlemnt to NHS services.  This information needs to be brought to the surgery for verification.

Our welcome pack includes a copy of the practice leaflet and practice charter. During the registration process you will be asked to complete a registration form for each family member including children.

Following registration your previous records will be requested, there is normally a delay of 4 to 6 weeks before thay arrive, except if urgent. The accuracy of information you provide us with during registration is therefore very important.
