New Patient Registration
St.Catherine's welcomes new patients, we have a straight forward registration process.
All registration applications will need to be accompanied by 2 forms of identification (1 needs to have a photograph i.e passport, driving licence and 1 needs to have confirmation of residency i.e utility bill). This will enable us to check your entitlement to NHS services. This information needs to be brought to the surgery for verification by the individual registering.
Our welcome pack includes a copy of the practice leaflet, medical health questionnaire and practice charters. During the registration process you will be asked to complete a registration form for each family member including children.
Following registration your previous records will be requested, there is normally a delay of 4 to 6 weeks before they arrive, except if urgent. The accuracy of information you provide us with during registration is therefore very important.
